Claim Forms
Please use the following word file to print off your claim form.
Step 1 After double clicking on the “Claim Form” link below, first select the type of claim you wish to make, Fire, Storm, Burglary.
Step 2 Secondly, choose your insurance company, e.g. Allianz, CGU etc.

You may then either
edit the claim form or print it off and
complete it by hand.
Please ensure all questions are fully answered. You should avoid using a dash. If something is not applicable say so. Post the fully completed and signed form back to our office with all relevant documentation including quotations, invoices, receipts, valuations and copy of police report or incident number.
Workers’ Compensation Claims As most claims need to be reported immediately to your insurer we recommend that you either call or fax them immediately with brief details of the loss or injury. They will give you instructions and mail out a claim form. Most companies allow you to report an injury and download a claim form from their website.
